Partners in Dementia Care: A Dementia Capable System of Care for Ohio Veterans
and their Caregivers

Implement and evaluate the effectiveness of an evidence-based care coordination program for veterans with dementia and their family caregivers delivered via a partnership between the Louis Stokes Cleveland VA Medical Center and the Western Reserve Area Agency on Aging and the Greater East Ohio chapter of the Alzheimer’s Association.
