What is Care Consultation?

Care Consultation is a telephone- and online-based program that assists family caregivers and adult care recipients, including those with chronic illnesses and disabilities, and the frail elderly.

Care Consultants are coaches working in partnership with care recipients, caregivers, other family members and providers. They specialize in:

• Assessing the Care Situation
• Identifying Current and Potential Challenges
• Providing Emotional Support
• Empowering Families through Action Planning
• Providing Information about Health Problems
• Mobilizing Families, Friends and Services

Care Consultants typically possess a bachelor's or master's degree in social work or nursing.
