Position: SOCIAL WORKER, LISW/GROUP FACILITATOR (25 hrs/wk)
Exempt Status: Exempt
Reports to: Director of the Adult Day Program and Partial Hospitalization Program
Responsibilities:

The Social Worker/Group Facilitator is a licensed independent social worker who will be responsible for conducting high quality, intensive, structured, and goal-oriented partial hospitalization therapeutic groups for older adults with severe mental illness and for completing all necessary documentation in a professional manner. This position functions under the direct supervision of the Assistant Director for Partial Hospitalization Services and is ultimately responsible to the Director of the Adult Day Program and Partial Hospitalization Program.

Qualifications:

Master's degree in social work from a social work department of a college/university accredited by the Council of Ohio Counselor, Social Worker, Marriage and Family Counselor Board. Clinical experience with older adults with SMD in an interdisciplinary setting preferred. Student or practice experience in clinical social work with the mentally ill preferred. Background check and drug test conducted. An insured car and valid Ohio driver's license required.

Position: ACTIVITIES ASSISTANT (20 hrs/wk)
Exempt Status: Non-Exempt
Reports to: Adult Day Program Director
Responsibilities:

The Activities Assistant is a member of the Adult Day Program team and leads and assists with program activities under the supervision of the Activities Coordinator. Clinical practice includes preparing and facilitating activities groups, evaluating participants' levels of functioning, and documenting on their progress toward established goals

Qualifications:

This position requires a bachelor's degree in recreational therapy, one of the creative arts, or a related field. ADC preferred. Certificate in Gerontology preferred. Knowledge of the aging process, skills necessary to work with mentally impaired older adults and minimum of one year work experience in an adult day care, assisted living facility or nursing home preferred. Drug test and background check required.


Position: REGISTERED NURSE – 40 hrs/wk and PRN Positions
Hiring Range: $37,440 - $54,402
Exempt Status: Exempt
Reports To: Director of Home Care
PRN Rate: Non-skilled: $30.00/Visit - $45.00/Assessment
Skilled: $40.00/Visit - $60.00/Assessment
Responsibilities: The Nurse is a member of the Home Care Department and is assigned clients on a per visit basis by the Director of Home Care or designee. The Nurse has the following responsibilities: Provision of nursing services to Home Health Care clients and their caregivers as assigned and supervision of Home Care Aides as requested.
Qualifications:

Current RN licensure in the State of Ohio required. Home heath care experience with older adults preferred. Strong interpersonal skills essential. Knowledge of Oasis, Medicare regulations and computer skills desirable. Works in a variety of settings with a culturally diverse population. An insured car and valid Ohio driver's license required.


Position: HOME CARE AIDE — Flexible (PRN)
Hiring Rates:

Weekday rate: $9.00
Weekend/Holiday Rate: $10.25

Exempt Status: Non-Exempt
Reports to: Director of Home Care
Responsibilities: The Home Care Aide, a paraprofessional worker, performs personal care and homemaking services in the homes of elderly clients in cooperation with and under the clinical supervision of a Community Services nurse or social worker. Serving both the east and west sides of Cuyahoga County.
Qualifications: High School diploma or equivalent with the ability to read, write and understand instructions. Must have home health certification or STNA including 75 hours of clinical training and be able to pass competency evaluation. Previous home care aide experience desirable. Must be able to communicate effectively with elderly clients and their caregivers. Reliable transportation helpful.

Position: PHYSICAL THERAPIST - PRN
Reports to: Director of Home Care
PRN Rate: $55.00/Visit - $65.00/Assessment
Responsibilities:

Responsible for providing physical therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Performs admissions and assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to physical therapist assistants, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient –focused interdisciplinary care team.

Qualifications: Graduate of an accredited Physical Therapy program with Bachelor's degree. Must be able to travel to patient's home. Must be able to read, write, and communicate with patients. Must be able to work in settings with infectious diseases. Must present a professional appearance at all times. At least 1 year clinical experience as a licensed physical therapist in an acute, rehab or skilled nursing setting. Current licensure in the State of Ohio . CPR Certification. Valid Ohio Driver's license.

Position: OCCUPATIONAL THERAPIST - PRN
Reports to: Director of Home Care
PRN Rate: $55.00/Visit - $65.00/Assessment
Responsibilities: Responsible for providing occupational therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to occupational therapist assistants, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient –focused interdisciplinary care team.
Qualifications: Graduate of an accredited Occupational Therapy program with Bachelor's Degree. Must be able to read, write, and communicate with patients. At least 1 year clinical experience as a licensed occupational therapist in an acute, rehab or skilled nursing setting. Current licensure in the State of Ohio . Current CPR certification. Valid Ohio Driver's license.

Position: HOMEMAKER AIDE - FLEXIBLE (PRN)
Hiring Rates: Weekday Rate: $8.00
Weekend/Holiday Rate: $8.20

Responsibilities:

The Homemaker is a paraprofessional member of the home care team trained to provide homemaking services in the home setting under the direction, instruction and supervision of a Registered Nurse, Licensed Practical Nurse or Social Worker. Serving both the east and west sides of Cuyahoga County .

Qualifications: High School diploma or equivalent with the ability to read, write and understand instructions. Must have at least one year employment experience as a supervised homemaker, and have successfully completed written testing prior to initiation of service provision. References, criminal background check and drug test required. Must be able to communicate effectively with elderly clients and their caregivers. Reliable transportation helpful.
