CONFERENCE OBJECTIVES

  Participants will

1. Gain an understanding of the range of age-related changes and other significant challenges to remaining at home, as well as at least six environmental modifications older adults can make to enhance their ability to stay home ,longer
2. Be able to describe components of a nursing home diversion model for high-risk mental healthy clients
3. Be able to describe a model of service integration among agencies with different missions and mandates
4. Gain increased knowledge regarding Medicare Part D and changes for 2010
