Goals of this project were:
To improve care for ABD/dual eligibles by identifying patterns in their use of primary, acute, and long-term-care services, associated costs, and issues in care management (e.g., medication noncompliance, lack of preventive care, poor outcomes at transition points).
To use this evidence base to improve the organization and delivery of care, including data-driven policy strategies that address current and emerging issues and possible solutions.
To assess the capacity of current service delivery system, especially in communities with large concentrations of high-risk populations and to examine the financial role played by Medicare, Medicaid and local funding.
To explore and implement new and sustainable care and financing alternatives, especially those that address the diverse needs of high-risk/high-cost populations.
