Social workers provide a range of services including, but not limited to:

• Assessment and re-assessment of the client
• Referral to community resources
• Nursing home and institutional placement assistance
• Assistance with client, family and team members with social, emotional and environmental issues relating to health needs
• Consultation with the client's physician regarding the client's status if his or her condition is adversely affected by social, environmental or economic conditions
• Coordination of interventions with the client's case manager
