Position: HOME CARE BEHAVIORAL HEALTH NURSE – East Side (PRN)
Exempt Status: Exempt
Reports to: Manager, Clinical Services
Responsibilities:

The Home Care Behavioral Health Nurse is a registered professional nurse who will provide psychiatric nursing services to Home Care clients and provides consultation regarding mental health services on a per visit basis. Responsibilities include: Provision of nursing services to reimbursable clients, Consultation and education to CS staff on mental health issues, Supervision of home care aides and students, Consultation and education to other community CS providers, Administration/program development, Research/Evaluation.

Qualifications:

BS in nursing or equivalent. Current RN licensure in the State of Ohio with 3-years as a professional nurse with 1-year in HHC or community nursing. Experience in psychiatric nursing; curriculum vitae must past authorization of Medicare Intermediary, Palmetto, to practice psychiatric nursing under Medicare/Federal Medicaid. Excellent assessment, communication, interpersonal, teaching and milieu management skills. Must be familiar with psychotherapeutic skills including family, behavior modification, cognitive therapy, stress management, case management and crisis intervention skills. Knowledge of Oasis, Medicare regulations and computer skills desirable. Works in a variety of settings with a culturally diverse population. Background and drug tests conducted. An insured car and valid Ohio driver's license required.


Position: MENTAL HEALTH CASE MANAGER - (40Hrs/Wk)
Hiring Range: $29,646 - $38,713
Exempt Status: Exempt
Reports to: Director, Mental Health Services
Responsibilities:

The Mental Health Case Manager will provide in-home Community Psychiatric Supportive Treatment (CPST) services. Experience in mental health social work with the elderly and knowledge of DSM IV preferred. Mental Health Case Managers are expected to use their cars in escorting clients when necessary.

Qualifications:

LPC, LPPC, LSW or LISW and current appropriate licensure in the State of Ohio preferred. License eligible BSW will be considered. Experience in clinical social work with culturally diverse elderly populations preferred. An insured car and valid Ohio driver's license required.


Position: SOCIAL WORKER (Part Time Flexible 20 Hrs/Wk)
Hiring Range: $15.00 - $18.00/hr
Exempt Status: Exempt
Reports to: Director, Clinical Services
Responsibilities: The Social Worker provides a wide variety of direct clinical services (individual and group) to community based elderly persons, families and/or other caregivers. The caseload assigned to the Social Worker will include those clients requiring professional skills integrating counseling, case management, and advocacy. The Social Worker is expected to draw upon considerable knowledge of the individual, the family and its social and cultural systems as well as the aging process, psycho/social theories, and functional/health issues. The Social Worker may supervise paraprofessional staff assigned to the team.
Qualifications:

BSW with one year experience or MSW from an accredited college/university and current social work license in the state of Ohio , LISW preferred. Experience in clinical social work with culturally diverse elderly populations preferred. An insured car and a valid Ohio driver's license required.


Position: REGISTERED NURSE – 40 hrs/wk and PRN Positions
Hiring Range: $37,440 - $54,402
Exempt Status: Exempt
PRN Rate: Non-skilled: $30.00/Visit - $45.00/Assessment
Skilled: $40.00/Visit - $55.00/Assessment
Responsibilities: The Nurse is a member of the Home Care Department and is assigned clients on a per visit basis by the Director of Home Care or designee. The Nurse has the following responsibilities: Provision of nursing services to Home Health Care clients and their caregivers as assigned and supervision of Home Care Aides as requested.
Qualifications:

Current RN licensure in the State of Ohio required. Home heath care experience with older adults preferred. Strong interpersonal skills essential. Knowledge of Oasis, Medicare regulations and computer skills desirable. Works in a variety of settings with a culturally diverse population. An insured car and valid Ohio driver's license required.


Position: HOME CARE AIDE — Flexible (PRN)
Hiring Ranges:

Weekday Rate: $8.50 (7:00 am and 6:00 pm weekdays)
Evening Rate: $9.75 (6:00 pm and 7:00 am weekdays)
Weekend/ Holiday Rate: $9.75

Exempt Status: Non-Exempt
Reports to: Director of Home Care
Responsibilities: The Home Care Aide, a paraprofessional worker, performs personal care and homemaking services in the homes of elderly clients in cooperation with and under the clinical supervision of a Community Services nurse or social worker. Serving both the east and west sides of Cuyahoga County .
Qualifications: High School diploma or equivalent with the ability to read, write and understand instructions. Must have home health certification or STNA including 75 hours of clinical training and be able to pass competency evaluation. Previous home care aide experience desirable. Must be able to communicate effectively with elderly clients and their caregivers. Reliable transportation helpful.

Position: PHYSICAL THERAPIST - PRN
Reports to: Director of Home Care
PRN Rate: $55.00/Visit - $65.00/Assessment
Responsibilities:

Responsible for providing physical therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Performs admissions and assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to physical therapist assistants, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient –focused interdisciplinary care team.

Qualifications: Graduate of an accredited Physical Therapy program with Bachelor's degree. Must be able to travel to patient's home. Must be able to read, write, and communicate with patients. Must be able to work in settings with infectious diseases. Must present a professional appearance at all times. At least 1 year clinical experience as a licensed physical therapist in an acute, rehab or skilled nursing setting. Current licensure in the State of Ohio . CPR Certification. Valid Ohio Driver's license.

Position: OCCUPATIONAL THERAPIST - PRN
Reports to: Director of Home Care
PRN Rate: $55.00/Visit - $65.00/Assessment
Responsibilities: Responsible for providing occupational therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to occupational therapist assistants, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient –focused interdisciplinary care team.
Qualifications: Graduate of an accredited Occupational Therapy program with Bachelor's Degree. Must be able to read, write, and communicate with patients. At least 1 year clinical experience as a licensed occupational therapist in an acute, rehab or skilled nursing setting. Current licensure in the State of Ohio . Current CPR certification. Valid Ohio Driver's license.
