Services are provided by a staff of licensed social workers and case manager assistants.
- Psychosocial assessment of economic, environmental, and social factors that interfere with the client's adjustment to illness and treatment.
Short Term Therapy for:
- Management of a terminal illness
- Reaction/adjustment to an illness
- Strengthening family/support systems
- Conflict resolution
Case Management and supportive counseling are provided for long range planning and decision making for the following problems:
- Adverse reaction or dysfunctional adjustment to illness and changes in functional status by the client or caregiver
- Barriers in access to medical care, including transportation
- Relationship problems including social isolation
- Caregiver stress and respite needs
- Placement issues
- Identification and education of community resources available to the client
- Specialized advocacy for at risk clients who have mental or physical limitations
