BRI Care Consultation Learning Collaborative – How to Leverage Partner Organizations to Implement the GUIDE Model

This session will answer some of your most burning questions about how to establish a, effective partnership with a Care Navigation partner – in this case between a healthcare system and a community-based organization — to deliver GUIDE Model services.
We’ll share the nuts and bolts of how Benjamin Rose, a Partner Organization, and Tembo Health, a virtual provider of specialty dementia care services and Participating Site, worked collaboratively to establish a division of responsibilities that address all GUIDE Care Delivery requirements. Our speakers will also share marketing opportunities, and the successes and challenges of recruiting eligible GUIDE beneficiaries into the program.
This will be an interactive session with lots of time for Q&A and discussion among attendees. We’ll also share details about our evidence-based program BRI Care Consultation, as well as the option for us to deliver care navigation services on your behalf through our WeCare…Because You Do delivery model.
