The Action Plan comes into play as soon as the first meeting with your client and is the driving force toward inspiring change in the care environment.   \n\n\n\nJoin us for an open-format BRI Care Consultation Learning Collaborative\, Creating a Five-Star Care Plan\, a guided discussion among Care Consultants and other professionals to learn how to develop a successful Action Plan\, including: \n\n\n\n\nHow to translate what was learned in the Assessment process into action steps\n\n\n\nTechniques to create action steps that are simple and realistic\n\n\n\nImportant elements to include in every care plan\n\n\n\nTips for increasing engagement and increasing task completion\n\n\n\n\nThis Learning Collaborative will not have a formal presentation; this is an opportunity to share and learn how to use the action planning process to your fullest advantage.
