Tembo Health and Benjamin Rose Join Forces to Enhance Dementia Care Through CMS GUIDE Model
Partnership supports National Dementia Awareness Month in November
Cleveland, OH – October 15, 2024 – Tembo Health, a leading provider of comprehensive clinical and medical care for dementia patients and Benjamin Rose, a renowned organization specializing in dementia care coordination and education, today announced a collaborative effort to improve dementia care delivery under the new CMS Guiding and Improved Dementia Experience (GUIDE) Model.
A bold new initiative from CMS, GUIDE is collaboration between medical practitioners and community Care Navigators that for the first time offers Medicare-covered services to address the unmet needs of people living with dementia and their family caregivers. GUIDE objectives include enhanced support for caregivers of people living with dementia, improved quality of life for people with dementia, and seamless coordination of services to enable people living with dementia to remain in the community.
Participants enrolled in GUIDE maintain their current primary care physician and health system; GUIDE services are layered on top of existing care to enhance clinical, behavioral and emotional supports.
This partnership leverages the strengths of both organizations to offer a holistic approach to dementia care. Tembo Health will contribute its expertise in diagnosing, treating, and managing dementia, providing cognitive services and personalized treatment plans. As a telehealth provider, Tembo’s virtual services offer convenience to busy caregivers and expanded access to families in remote areas. Benjamin Rose will provide essential care coordination delivered by phone and email, including family education and community support services, such as caregiver support groups and respite care options.

The Centers for Medicare & Medicaid Services (CMS) launched the GUIDE Model in an effort to improve care coordination, patient outcomes, and caregiver well-being for Medicare beneficiaries with dementia. By participating in the model, Tembo Health and Benjamin Rose aim to demonstrate the effectiveness of their combined approach in delivering high-quality, person-centered dementia care.
