Please join us for this BRI Care Consultation Learning Collaborative, Connecting the Dots: How Community-Based Organizations Strengthen Comprehensive Dementia Care, presented by Kerrie Roth, MSW, LSW, Clinical Manager of Aging and Disability Resource Center, Western Reserve Area Agency on Aging.

This informative and timely presentation will provide insights into the roles and functions of community-based organizations, including Area Agencies on Aging and multi-service organizations, as well as providers of Adult Day, transportation, respite and home health.

In this session, you’ll learn:

  • The home-and community-based services these providers offer to people living with dementia and their caregivers, including caregiver education and skills-training, help with ADLs, case management, home health, hospice, and respite
  • How the referral process can work
  • Ways to partner with community-based organizations

This is an excellent opportunity for healthcare providers and aging services organizations to get an in-depth understanding of the support services and resources community organizations can offer, and how their services fully integrate into comprehensive dementia care.
