This 30-minute presentation, followed by Q&A, will include an overview of CMS’ objectives for developing GUIDE, a review of the 9 required elements of GUIDE delivery, including clinical, care navigation, respite and other services, eligibility parameters, as well as the unique role that older adult services and healthcare organizations can play in the process. You’ll come away with a clear understanding of what the Ohio landscape will look like. More importantly, you’ll learn the role that AAAs, community-based organizations and health providers can play in referring clients and receiving referrals that will increase usage of your current programming. Benjamin Rose Institute on Aging is an active participant in GUIDE service delivery through our evidence-based BRI Care Consultation program. We’ve partnered with a number of health providers to deliver the care navigation elements of the GUIDE program.
