Many families facing dementia struggle to find the care, support and information they really need. Medicare’s new GUIDE program is designed to solve this challenge by providing access to medical specialists and assistance in guiding you through the complexities of the healthcare system.

WeCare delivers GUIDE services to dramatically improve dementia care
We’ve partnered with national telehealth providers to deliver state-of-the-art medical care and clinical services to patients. Through this partnership, you’ll have access to:
- A 24-hour helpline that connects you with experts who can answer your questions. In times of urgency, you can speak with a doctor within minutes
- Up to $2500 annually in respite care, giving you time to manage other responsibilities and recharge
- A personal Care Navigator to help coordinate both healthcare and supportive services, guiding you to the resources and programs right for you
- Education about the disease and how to manage challenging behaviors
One phone call for all of your needs
With the GUIDE program from WeCare, you will be connected with your personal Care Navigator, who will be your single contact for all the services you need. Your Care Navigator will create a personalized care plan, find right-fit programs and services, help you improve your knowledge and skills, and to help you stay physically and emotionally fit. Who can participate in GUIDE?
Eligibility for the no-cost GUIDE program is simple. The participant must:
-Be experiencing some degree of cognitive impairment – but does NOT need a formal diagnosis
-Be enrolled in Medicare Part A and B
-Not be enrolled in hospice or PACE

When George was diagnosed with dementia, his wife Maria felt lost. “I wanted to make sure that he was well taken care of, but didn’t know how to do it on my own. Then I heard about WeCare, and met our wonderful Care Navigator, Lauri.”

Month 1
- On the very first call, I discussed my top concern – my fear that I was not meeting George’s needs.
- Lauri and I developed a personalized list of simple steps to meet my needs, giving me a place to start and a path forward.
- Lauri scheduled an appointment with a dietician to review George’s nutrition.
- I can feel the weight start to lift off my shoulders.
Month 2
- Lauri found an expert to assess our home for safety. They installed grab bars in the bathroom and lowered shelves in the kitchen.
- We enrolled George in a day program, providing him with an opportunity for socialization.
- Lauri helped me to see that I can use this time for me to enjoy my own hobbies.
- George is now receiving the care and support I would like him to have.


Month 3
- Lauri and I worked on a plan for the future.
- My daughter helped me to hire a home health aide to assist with George’s morning and evening routines.
- I joined a support group to learn from other caregivers and benefit from the emotional support.
- I am calmer, knowing the right support services are in place.
