- Comprehensive care coordination to address the physical, emotional and behavioral needs of the caregiver and the patient
- Techniques to enhance the physical and emotional well-being of the family caregiver
- Improved quality of life for people living with dementia or other conditions
WeCare will also determine your patient’s eligibility for the CMS GUIDE program, which will provide enhanced medical care and all WeCare supports at no cost to them, including:
- 24-hour access to doctors for medical advice and emergency assistance
- A comprehensive dementia assessment and diagnosis
- Up to $2,500 in respite dollars annually
