This session will explore the key principles of quality dementia care navigation and the critical role that partnerships between healthcare systems and community organizations play in enhancing care for people living with dementia and their care partners. Attendees will learn about strategies for developing and sustaining effective collaborations that: 1) address the needs of people living with dementia and their care partners, 2) improve resource accessibility, and 3) promote holistic care approaches by highlighting a successful collaboration model between the Cleveland Clinic Foundation and Benjamin Rose.
